Players will not be ALLOWED TO PRACTICE until registration, paperwork, and payment are completed in full!

Tackle Football Paperwork Nights are Thursday, July 23 and Thursday, August 1 from 5-7pm at Waltham Boys & Girls Club (20 Exchange Street). The last day to submit tackle football paperwork or payment is Thursday, August 1, 2024.  

Flag Football Paperwork Nights are Thursday, August 1 and Wednesday, August 14 from 5-7pm at Waltham Boys & Girls Club (20 Exchange Street). The last day to submit flag football paperwork or payment is Wednesday, August 14, 2024.

PAPERWORK REQUIREMENTS:

1.  Image Release - Minor

2.  Waiver and Release of Liability - Minor

3. Emergency Medical Treatment, Consent and Information

4. Participant Page 1 - you do not need to provide a photo; we will take photos in uniform jersey for this

5. Participant Page 2 

6. AYF Code of Conduct - signed by player and parent 

7. Concussion Acknowledgement - signed by player and parent 

8. Copy of 2024 Physcial (dated between 1/1/24 and 7/31/24 ).  All players must have a doctor sign the medical clearance form if after July 31, 2024.  Books will be certified by AYF mid-August so unfortunately NO EXCEPTIONS. 

9. Birth Certificate (new players). Returning players should confirm their birth certificate is on file with WYF&C by emailing Erin Donovan at erinmarie1122@gmail.com.

10. 2023-2024 Report Card (June with all semesters)

The following forms are not required: Absentee Form and Resume Participation Medical Clearance Form
