2025 PAPERWORK REQUIREMENTS

Players and cheerleaders will not be ALLOWED TO PRACTICE until registration, paperwork, and payment are completed in full!

The last day to submit cheer paperwork or payment is Monday, June 30, 2025.  Cheer paperwork must be submitted to Stephanie Krol or Karen Pino.

The last day to submit tackle football paperwork or payment is Thursday, July 31, 2025.  

The last day to submit flag football paperwork or payment is Thursday, August 14, 2025.

PAPERWORK REQUIREMENTS:

1.  Image Release - Minor

2.  Waiver and Release of Liability - Minor

3. Emergency Medical Treatment, Consent and Information

4. Participant Page 1 - you do not need to provide a photo; we will take photos in uniform jersey for this page.

5. Participant Page 2 

6. AYF Code of Conduct - signed by player AND parent 

7. Concussion Acknowledgement - signed by player AND parent 

8. Copy of 2025 Physcial (dated between 1/1/25 and 7/31/25).  All players must have a doctor sign the medical clearance form if physicals are after July 31, 2025.  Books will be certified by AYF mid-August so unfortunately NO EXCEPTIONS. 

9. Birth Certificate (new players). Returning players should confirm their birth certificate is on file with WYF&C by emailing Erin Donovan at erinmarie1122@gmail.com.

10. 2024-2025 Report Card (June with all semesters)

11. Abesntee Form - only required for CHEER. Signature only, do not date. 

The following forms are not required: Resume Participation Medical Clearance Form and Absentee Form for Football
